The quote is free, but as you may have guessed, the application process requires some effort. So we have tried to make it as simple as possible.

On this page, you will find all the forms you’ll need, including:

Also, as part of the free quote, you will have to provide the following documents:

  1. If you are a Group (applicant with payroll) – UC2A form must accompany application (if employee is new and not on the last UC2A, payroll records must be included).
    If you are a Partnership – K1 tax return for each partner (if partnership has just begun, a copy of the partnership agreement must be presented).
    If you are a Sole Proprietor – Schedule C of the previous year Federal income tax return must accompany the application.
    If you are an S-Corporation – Copy of the 1120 Federal income tax return for previous year (1st page only)
  2. A copy of your health insurance invoice.
  3. Renewal notice from current carrier (if applicable).
  4. Copy of design sheet for current coverage (if applicable).
  5. Signed “Master Application.”
  6. Completed application for each full-time employee on payroll.  If waiving coverage, a form must be completed and signed to waive the coverage.
